Nurse Navigator – Patient Navigation RN

LifePoint Health seeks a Nurse Navigator to guide patients through their care journey in our community-based facilities. In this role, you will serve as a central point of contact, coordinating appointments, referrals, and services while ensuring timely access to care. You will assess patient needs, provide education, support transitions between settings, and collaborate closely with physicians, nurses, and allied health teams to improve quality, safety, and outcomes.
