Minimum qualifications
- Bachelor'sdegree in Nursing
- CurrentStateofMichiganRegisteredNursingLicense.CPRorBCLSCertified.
- Three years of experience in an acute care setting. One year of experience in medical, surgical or radiationoncology(foroncologyspecialty).
What the Nurse Navigator will do:
- Completes a comprehensive health assessment of the patient's physical, psychological, social, environmental,financialandfunctionalstatusincollaborationwithmultidisciplinaryteammembers.Includesassessmentofpastandpresenttreatmentcourse.
- Assistsinorganizingandcoordinatingtimelypatientappointments.
- Understands and develops goals, treatmentplanand provider options in concordance withevidence basedstandards and clinicaltrials.
- Worksdirectlywithphysicianleadtoreviewdiagnosticstudies,clinicalhistoryandcourseoftreatmenttoidentifymostappropriatenextcourse ofcare.
- Collaborateswithmultidisciplinaryteammembersinperformanceimprovementactivitiesincludingassessment,planning,implementationandevaluationofclinicaloutcomesand processesfor patients.
- Usescritical thinking andproblem solvingskills to analyze and synthesize data and formulatean appropriate casemanagementplantoaddressthe patient'sindividual andfamilyneeds.
