RiverRidge Outpatient - ME seeks a Nurse Navigator to support clients in our outpatient behavioral health programs. In this patient-centered role, you will coordinate care, perform nursing assessments, and guide individuals and families through treatment, resources, and follow-up services. Working within a collaborative, multidisciplinary team, you will help link patients to counseling, psychiatry, and community supports, promote engagement in care, and monitor progress.
