Houston Methodist seeks a compassionate Nurse Navigator to guide patients and families through complex care journeys. In this role, you will coordinate appointments, tests, and referrals, provide education and emotional support, and act as a key liaison between patients, physicians, and the interdisciplinary care team. You will assess barriers to care, promote safety and quality, and ensure timely access to services across the continuum.
