Weekend Nurse (LPN/RN)
Location: Carroll, Iowa
Schedule: Weekends 2 on 1 off
Day position 6:00a to 2:00p
Evening position 2:00a to 10:00p
New Hope is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team caring for individuals with disabilities. This position is ideal for a nurse who values consistency, teamwork, and the opportunity to build meaningful relationships with our individuals while providing high-quality nursing care in a supportive environment.
Position Summary
The LPN/RN is responsible for providing direct nursing care, monitoring resident health status, administering medications and treatments, and working closely with the interdisciplinary team to ensure residents receive safe, compassionate, and person-centered care. This role plays a key part in supporting the health, comfort, and quality of life of individuals living in our ICF setting.

Essential Duties and Responsibilities
- Provide skilled nursing care to residents in accordance with physician orders, care plans, and facility policies
- Administer medications, treatments, and other nursing interventions accurately and safely
- Assess, monitor, and document resident conditions, responses to treatment, and changes in status
- Communicate changes in resident condition to providers, family members, and the care team as appropriate
- Collaborate with direct support staff, therapists, and other interdisciplinary team members
- Ensure accurate and timely charting in the medical record
- Promote resident dignity, independence, and quality of life
- Follow all state, federal, and facility regulations related to resident care, infection control, and safety
License & Certifications
- Unencumbered RN/LPN license in Iowa
- Valid driver's license and chauffeur's license (D3) with a good driving record
- Ability to complete the Safety Care Training Course
