Program Description
REACH is a system of supports for individuals (adults and youth) with developmental disabilities who are at risk of crisis. REACH offers 24/7 mobile crisis response, community crisis stabilization, residential crisis stabilization and crisis prevention services. The program also operates an adult transitional home. The homes are all located within Chesterfield County. In addition to the crisis services, REACH also provides system training and consultative support. RBHA operated REACH.
Job Summary
Provides nursing care and clinical support to individuals with behavioral health, developmental, substance use, and/or medical needs. Performs health assessments, administers medications and treatments, monitors client health status and response to care, maintains clinical documentation, and collaborates with interdisciplinary treatment teams to promote safe, effective, and coordinated care. Provides client education, supports continuity of care, and performs nursing functions consistent with applicable licensure, established clinical protocols, provider orders, and organizational policies and procedures. The level of clinical independence, complexity of assignments, and leadership responsibilities may vary based on position level, licensure, experience, and program needs

- Provides nursing care and clinical support in accordance with applicable standards of practice, scope of licensure, provider orders, and established policies and procedures.
- Administers medications, injections, and other prescribed treatments as appropriate within scope of practice; monitors and documents client response, medication adherence, effectiveness, and potential side effects.
- Conducts nursing and health assessments, obtains and monitors vital signs, evaluates client health status, and identifies changes or concerns requiring intervention, referral, or escalation.
- Performs or assists with clinical procedures and collection of laboratory specimens, as appropriate to the position and scope of practice.
- Maintains accurate, complete, and timely clinical documentation in electronic health records and other required systems.
- Collaborates with physicians, providers, clinicians, case managers, pharmacies, and other interdisciplinary team members to coordinate treatment and support continuity of care.
- Participates in treatment planning, interdisciplinary team meetings, admissions, discharges, and transitions of care as assigned.
- Educates clients regarding medications, health conditions, treatment recommendations, wellness practices, and other health-related needs.
- Responds to client health concerns and assists in determining appropriate nursing interventions or level of care within the scope of the position.
- Coordinates or assists with medication access, pharmacy communication, insurance-related needs, and other healthcare resources as appropriate.
- Monitors, orders, organizes, and maintains medications, medical supplies, and clinical equipment in accordance with established procedures.
- Maintains safe, organized, sanitary, and appropriately stocked clinical work areas.
- Supports crisis response, de-escalation, and other interventions necessary to maintain client and staff safety.
- Communicates significant changes in client condition, treatment response, or other clinical concerns to appropriate members of the treatment team.
- Adheres to applicable nursing standards, medication protocols, documentation requirements, safety practices, confidentiality requirements, and regulatory standards.
- Participates in quality improvement, staff development, training, and program initiatives as assigned.
- May provide clinical guidance, training, or support to other staff consistent with position level and organizational needs.
- Performs other duties as assigned
Minimum Qualifications-Education and Experience
- Graduation from an accredited or approved nursing education program appropriate to the required level of nursing licensure.
- Nursing experience in behavioral health, mental health, medical, psychiatric, residential, substance use treatment, long-term care, or a related healthcare setting, as required by the level of the position.
- Current applicable nursing license issued or recognized by the Virginia Board of Nursing, appropriate to the position.
- CPR and CPI certifications, as required.
- OR an equivalent combination of education, experience, knowledge, skills, and abilities that demonstrates the capacity to perform the essential functions of the position, as permitted by applicable licensing and regulatory requirements.
- Possession of a valid Virginia driver's license and a satisfactory driving record in accordance with agency policy, when required by the position.
- Experience providing nursing services in behavioral health, psychiatric, substance use treatment, detoxification, residential, long-term care, or other relevant healthcare settings.
- Experience using electronic health record systems.
- Training or experience in trauma-informed care and motivational interviewing.
- Experience coordinating interdisciplinary care for individuals with complex behavioral health and medical needs.
- Leadership, mentoring, or staff training experience for positions requiring advanced nursing responsibilities.
- Knowledge of professional nursing standards, clinical practices, medication administration protocols, and applicable health and safety requirements.
- Knowledge of behavioral health and medical conditions and their impact on client care.
- Knowledge of applicable documentation, confidentiality, safety, and regulatory requirements.
- Skill in conducting health and nursing assessments and recognizing changes in client condition.
- Skill in administering medications and treatments accurately and safely within applicable scope of practice.
- Skill in communicating effectively with clients, families, healthcare providers, and interdisciplinary team members.
- Skill in documenting clinical information accurately, clearly, and timely.
- Ability to recognize potential clinical or safety concerns and take or initiate appropriate action consistent with scope of practice.
- Ability to organize and prioritize responsibilities while managing multiple client and program needs.
- Ability to follow established clinical protocols, provider orders, organizational policies, and regulatory requirements.
- Ability to work independently within established scope and appropriately seek consultation, direction, or escalation when needed.
- Ability to maintain professional boundaries and exercise sound clinical judgment.
- Ability to respond effectively to urgent, emergent, or crisis situations.
- Ability to learn, retain, and apply clinical and organizational procedures.

Work Environment and Physical Demands
This role, generally performs work under the following conditions:
- Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects, including the human body. While lifting demands may vary, employees in this category must demonstrate consistent mobility and physical readiness to perform assigned duties. This may include operating a motor vehicle, walking extended distances, climbing stairs, and navigating uneven terrain.
- Requires close concentration and visual attention to safely perform clinical duties, administer medications, complete documentation, and monitor client health and safety.
- Work is performed in variable clinical, residential, community-based, and/or behavioral health environments depending on assignment.
- May be exposed to potentially hazardous conditions associated with healthcare delivery, including microorganisms, bloodborne pathogens, bodily fluids, bacteria, and individuals or animals presenting potential safety risks.
- May encounter clients experiencing behavioral health crises, including aggressive, self-injurious, or otherwise unsafe behaviors. The position may require implementation of approved crisis prevention, de-escalation, behavioral intervention, and emergency response procedures consistent with training, organizational policy, and the employee's assigned responsibilities.
- Requires the physical ability to perform nursing and client-care activities associated with the position and to respond appropriately to urgent or emergency situations.
- May require standing, walking, bending, reaching, positioning, or assisting clients and transporting or handling medical supplies and equipment.
- May require travel between agency, residential, clinical, community, or other service locations depending on assignment.
