nRole Overview MICare Management is seeking a compassionate and driven Registered Nurse (RN) Case Manager to join our growing team. This is a full-time hybrid role based in Canton, MI, offering flexibility for remote work.
nIn this role, you will guide patients through the healthcare system, proactively address potential medical concerns, and coordinate personalized care plans for individuals with chronic and complex conditions. You’ll play a critical role in improving outcomes while building meaningful patient relationships.
nKey Responsibilities
nNavigate and coordinate care across healthcare systems
nIdentify and prevent potential medical complications
nDevelop and implement individualized care plans
nSupport patients with chronic and complex conditions
nCollaborate with providers, families, and interdisciplinary care teams
nMaintain accurate documentation and ensure compliance with healthcare regulations
nRegistered Nurse (RN) Requirements
nActive and unrestricted RN license in the state of Michigan
nMinimum of 1–2 years of clinical experience (case management, home health, rehab, or acute care preferred)
nStrong understanding of care coordination, discharge planning, and chronic disease management
nKnowledge of healthcare regulations, documentation standards, and compliance requirements
nExperience with EMR/EHR systems
nExcellent clinical judgment and critical thinking skills
nStrong communication and patient education abilities
nAbility to work independently, prioritize tasks, and manage a flexible schedule
nValid driver’s license and reliable transportation
