Providing telephonic care management for members with complex and chronic care needs, the full-time Telephonic Nurse Case Manager I will assess, develop, implement, coordinate, monitor, and evaluate care plans while working remotely with occasional in-person training sessions. Key responsibilities: Conduct assessments to identify individual needs and develop specific care management plans Implement care plans by facilitating authorizations and coordinating internal and external resources Monitor and evaluate the effectiveness of care management plans, modifying them as necessary Required qualifications: BA/BS in a health-related field with a minimum of 3 years of clinical experience, or equivalent combination of education and experience Current, unrestricted RN license in applicable state(s) Multi-state licensure required for servicing members in different states Certification as a Case Manager is preferred
