Empowering families for optimal wellness, the full-time Texas Licensed Nurse Case Manager will work remotely to assess member needs, develop individualized care plans, and coordinate necessary medical services for high-risk populations in a telephonic managed care setting. Key responsibilities Assess physical, functional, psychological, and environmental needs of members and develop tailored care plans Monitor and adjust care plans to ensure appropriate service provision and benefit adequacy Interface with healthcare providers to coordinate care activities and provide education to members about their treatment plans Required qualifications Current RN licensure in Texas or Compact licensure Three or more years of clinical experience in a Level III or above NICU CCM certification required within specified timeframe Experience in a Case Management role and familiarity with maternity management programs preferred Proficiency in data entry into databases or EHR systems
