Nurse Case Manager – Clinical Services

Physicians Data Trust is seeking a skilled Case Manager to support members in case management within a managed care framework. The role requires collaboration with Health Plan and Hospital Case Management, physicians, and IPA Medical Directors to facilitate appropriate healthcare and services under Medicare and Medi-Cal programs.

Qualified candidates have 2-3+ years in case management, an active LVN license in California, and strong communication and regulatory knowledge.
