Nurse Case Manager: Care Continuum Navigator

Ascension in Panama City, FL is seeking a Case Management RN to orchestrate the plan of care from admission to discharge across the continuum. You will assess complex needs, advocate for patients to access internal and community resources, and guide post-discharge planning with the care transition team.

The role requires Florida RN licensure; CCMC is preferred. Diploma from an accredited nursing program or equivalent licensure is required.
