Managing patients' specialized needs remotely, the full-time Nurse Care Manager will take ownership of patients with complex chronic conditions, collaborating with healthcare providers to develop comprehensive nursing care plans aimed at optimizing patient outcomes and avoiding preventable admissions. Key responsibilities Quickly build empathetic relationships with patients and families while evaluating their needs based on individual values and preferences Interface with healthcare providers to develop and implement case management plans, coordinating access to health services across multiple disciplines Monitor and evaluate the effectiveness of nursing care plans, making necessary modifications to ensure quality and cost-effectiveness Required qualifications Active Registered Nurse license in New York and/or Michigan Experience engaging patients virtually using various technologies Bachelor's of Science in Nursing preferred 3+ years of clinical practice in a healthcare setting highly preferred Prior experience with electronic health records or health registries required
