School Nurse

Accredited college or university degree, preferably baccalaureate degree and state licensure as an RN. Knowledge and experience in a school health related area. Working knowledge and skill in the successful treatment and care of students within the school corporation. Communicate well and be able to successfully work with students, staff, teachers and principal.

To protect and promote student health, facilitate optimal development and academic success by promoting a safe and healthful environment for all students, faculty, staff and administration by applying principals of healthful living to the school program.

Maintain student health records through the course of the school year and check all student health records to ensure all required immunizations are complete and recorded. Compile data and retain to assist and complete the required School Health Report to the Indiana Department of Education which includes the immunizations, vision and hearing screens. Gives immediate care to injured or ill students, notifies parents, advises and encourages parents on extended care and treatment, assists parent in arranging for transportation to home or hospital. Conduct vision screening for grades 1, 3, 5, and 8 according to Indiana State Law requirements. Students who fail the screening test are given referral notices recommending further examination by their eye physician. Conduct hearing screens on all new students to the corporation and on grade levels 1, 4, 7, & 10 according to Indiana state law requirements. Students who fail the screens are given referral notices recommending further exam by their physician. Exclude from school those students that present signs and symptoms of a communicable disease according to the public health code of the Indiana State Board of Health. Assist with the coordination of kindergarten registration, prepares and distributes necessary form for registration; medical and dental examination. A school accident form is completed by the school nurse for any student as needed. Assists parents or guardians in obtaining financial aid from civic organizations for glasses and dental care. Prepares a Children and Hoosier Immunization Registry Record (CHIRP) on all new, incoming students to the corporation students, including kindergarteners utilizing the information given to physician as to present health status, immunizations and special testing. Maintains this health record throughout the school year. Send a health report with pertinent information on students transferring from Crown Point Community School Corporation including immunization Implement care coordination by providing case management, chronic disease management, education, counseling, nursing delegation, care plans, student self-empowerment, & transition planning. Implement quality improvement by documentation/data collection, evaluation, & research. Supports community/public health by providing access to care, cultural inclusiveness, disease prevention, health education, health promotion, risk reduction, & screening/referrals/follow-up. All other health services related duties as assigned by the principal.
