NP: Transition & Primary Care (Outpatient, Full-Time)

MVHC is seeking a Nurse Practitioner to join our Transition of Care/Primary Care team in Zanesville, OH. This full-time role focuses on outpatient care with no on-call requirements and shared responsibilities across medicine and care coordination.

You will conduct post-discharge transitions, perform assessments, and develop individualized care plans while collaborating with physicians, nurses, and social workers in a supportive, patient-centered environment. Epic EMR is used for documentation.
