Key responsibilities Identify and address gaps in preventive care and chronic disease management for patients with substance use disorders Provide patient education and promote self-management through evidence-based care plans Coordinate transitions of care to ensure high-quality and efficient healthcare services Required qualifications BSN or ADN degree from an accredited school or college of nursing Current RN license in North Dakota, Minnesota, and/or Wisconsin, with the ability to obtain all three within three months of hire Experience in addiction care management or related fields is preferred Knowledge of regulatory requirements related to substance use disorder care Ability to work collaboratively within an interdisciplinary team
