Providing nursing support to members with low-acuity, short-term clinical needs, the full-time North Carolina Licensed Resource RN will work remotely to conduct transitions of care assessments, perform clinical education, and evaluate members for higher levels of care management. Key Responsibilities Conduct focused transitions of care assessments for members during inpatient stays or post-discharge Perform self-efficacy and condition-specific screenings, including behavioral health tools, to identify members needing additional support Collaborate with care team members to ensure proper medication reconciliation and address quality gaps in care Required Qualifications Active North Carolina RN license or Compact RN license valid in North Carolina Graduate of an accredited school of nursing (R.N.) Minimum of 3 years of nursing experience Familiarity with North Carolina communities and healthcare resources Experience applying Motivational Interviewing and Trauma-Informed Care principles
