To support adult members transitioning from Skilled Nursing Facilities, the full-time salaried North Carolina Licensed Registered Nurse will perform nursing assessments, care planning, and coordination of services remotely while collaborating with various healthcare providers and community stakeholders. Key responsibilities Conduct nursing assessments and coordinate care plans for members transitioning out of Skilled Nursing Facilities Collaborate with healthcare providers and community partners to ensure comprehensive service delivery and address barriers to care Maintain timely documentation of member interactions and care coordination efforts Required qualifications Associate Degree in Nursing with a valid North Carolina Registered Nurse license Minimum of two years of experience as a Registered Nurse, preferably in a skilled nursing facility Knowledge of community resources and healthcare systems, including Medicare and Medicaid Clinical knowledge of the Minimum Data Set (MDS) System and care strategies for activities of daily living Ability to effectively communicate and collaborate with diverse healthcare teams and stakeholders
