To support cancer patients through their care journey, the full-time North Carolina Licensed Oncology Nurse Navigator will manage treatment plans, facilitate transitions from inpatient to outpatient care, and provide education and resources, working in a hybrid environment that includes both remote and in-person responsibilities. Key responsibilities Facilitate timely coordination of appointments, diagnostic testing, and procedures for oncology patients Provide individualized assistance and education to patients, families, and caregivers throughout the cancer care continuum Assess patients' health practices and compliance with treatment plans, documenting findings and coordinating care as necessary Required qualifications Bachelor's degree in Nursing (BSN) from an accredited school, or an ADN with a commitment to obtain a BSN within specified timeframes Licensed to practice as a Registered Nurse in North Carolina OCN or ONN Certification required within three years of hire Minimum of two years of nursing experience, with at least one year in oncology-related care Basic Life Support (BLS) for Healthcare Provider certification
