North Carolina Licensed NP/PA

To support a mission-driven interdisciplinary clinical team, the full-time North Carolina Licensed NP/PA will conduct telehealth video visits, perform comprehensive assessments, and emphasize preventative care while collaborating with various healthcare professionals in a fully remote environment. Key responsibilities Conduct primarily telehealth video visits and occasional in-home visits to provide comprehensive assessments and evidence-based care Identify care gaps and coordinate with care team members to develop individualized, whole-person care plans Utilize integrated technology to enhance member interaction and reduce administrative burdens Required qualifications Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license 3+ years of outpatient clinical practice, preferably in primary care, family medicine, internal medicine, or geriatrics Active BLS certification at time of hire Comfort with delivering care via video telehealth and using electronic documentation systems Willingness to obtain additional licenses if necessary within the first 90 days
