To support a leading organization in the health insurance and managed care industry, the fully remote North Carolina Licensed Case Manager will conduct comprehensive evaluations, coordinate services with interdisciplinary healthcare teams, and monitor member progress to improve outcomes and promote quality care. Key responsibilities Conduct comprehensive evaluations to identify member needs and create individualized care strategies Partner with members, families, and healthcare providers to coordinate services across the healthcare continuum Monitor member progress through outreach and care plan updates to ensure optimal clinical outcomes Required qualifications Active RN, RD, or LCSW license with a minimum of three years of clinical, care coordination, or case management experience Master's in Social Work with five years of related experience may be considered for select positions Behavioral Health-focused candidates may qualify with alternative applicable licensure and at least three years of direct clinical experience Must possess and maintain a valid North Carolina or compact multi-state clinical license in good standing Strong assessment, care planning, and interdisciplinary collaboration skills required
