Bay Area Community Health (BACH) is seeking a Community Health Worker for a full-time, permanent role supporting clinic operations in Glenview and Gilroy . - Monday: 10:00 AM-7:00 PM
- Tuesday: 8:00 AM-5:00 PM
- Wednesday: 8:00 AM-5:00 PM
- Thursday: 8:00 AM-5:00 PM
- Friday: 8:00 AM-5:00 PM
- Monday: 10:00 AM-7:00 PM
- Hours per Week: 40
- Contract Length: Permanent (no end date)
- Start Date: ASAP
- Charting System: EPIC
- Interview: Amergis conducts interviews (Teams, 30 minutes, camera on)
- Compassion and desire to work in community-based settings serving low-income families
- CHW certification required
- Strong customer service skills
- Knowledge of local community resources
- Candidates with prior BACH employment will not be considered

Minimum Requirements:
- Professional License in Behavioral Sciences, Human Services, or Social Services preferred
- Bachelor’s Degree in Behavioral Sciences, Human Services, or Social Services fields preferred
- Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred
- Complies with all relevant professional standards of practice
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
