Gentiva Hospice invites an experienced Afterhours RN Care Manager to join our team and oversee clinical care outside regular hours. You will perform after-hours visits, admissions, and death visits, while coordinating with physicians and the interdisciplinary group to ensure seamless patient care.
You will assess needs across physical, emotional, psychosocial, and spiritual domains, manage plans of care, and guide field staff with strong nursing leadership and compliance responsibilities.
