Night-Shift Hospice Care Manager RN

Heartland Hospice is seeking an Afterhours RN Care Manager to provide high-quality, compassionate hospice care during nights, weekends, and on-call hours. You will ensure seamless coverage, continuity of care, and strong clinical oversight for patients and families.

Responsibilities include direct patient care during after-hours shifts, assessments, plan of care updates with physicians and the IDG, education for families, and maintaining timely documentation.
