To support kidney health management, the remote New York RN Care Navigator will manage a caseload of patients, facilitating care coordination, conducting health screenings, and developing member-centric care plans while ensuring high-quality outcomes. Key responsibilities Handle inbound and outbound calls to provide exceptional service and education regarding kidney health and related conditions Engage and follow up with members in HealthMap's Kidney Health Program, addressing their needs and barriers to care Collaborate with interdisciplinary teams to coordinate care and advocate for patients' individualized goals Required qualifications Active, unrestricted RN license in the state of New York Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of a degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis center or transplant center preferred
