New York Licensed Social Care Navigator

Supporting New York's Medicaid Section 1115 Waiver program, the remote Bi-Lingual Spanish/English Social Care Navigator will manage member communications, assess eligibility for services, and coordinate referrals while ensuring compliance with Medicaid and HIPAA regulations. Key Responsibilities Manage inbound and outbound member communications via phone and email using various technology platforms Conduct outreach to screen potential members and navigate them to appropriate service providers based on health-related social needs Track and follow up on referrals to ensure service delivery and maintain accurate documentation in internal systems Required Qualifications High school diploma (Associate degree preferred) 3-5 years of experience in customer service, call centers, social services, or care navigation Experience in a virtual contact center environment (Five9 preferred) Strong communication and member engagement skills Ability to manage high-volume interactions in a compliance-driven setting
