To support a growing population health team, the full-time New York Licensed RN Case Manager will coordinate comprehensive patient care, conduct assessments, and collaborate with multidisciplinary teams, all while working remotely. Key responsibilities: Conduct initial intake calls and ongoing outreach to assess patient needs and update care plans Provide care coordination for a panel of complex patients, ensuring alignment with their needs and wishes Collaborate with the care team to identify interventions, address gaps in care, and support patients during transitions Required qualifications: Current RN License in good standing in New York or an unencumbered multistate license Bachelor's degree in nursing or an associate's degree in nursing with another bachelor's degree Minimum of 4 years of clinical experience, including at least 2 years in case management Certified Case Manager (CCM) certification or commitment to obtain it when eligible Experience in geriatric care and knowledge of Medicare and Medicaid regulations preferred
