To support patients facing advanced illness, the full-time New York Licensed Palliative Care RN will provide home-based palliative symptom assessment, treatment, and ongoing goals of care discussions while collaborating with an interdisciplinary team. Key responsibilities Assess and address the complex care needs of patients and families in their homes Coordinate care plans and provide supportive counseling to reduce anxiety and symptoms related to advanced illness Engage patients and caregivers in discussions about goals of care and treatment plans Required qualifications Graduation from an accredited Nursing program; BSN or ADN with a plan to complete a BSN within the year Current New York State RN license 2+ years of experience managing patients with complex and chronic illnesses Relevant clinical competence in nursing practice Basic Life Support (BCLS) certification from AHA
