New York Licensed Palliative Care Nurse

To support a growing practice, the full-time salaried New York Licensed Palliative Care Nurse Navigator will engage with members experiencing serious illness, conducting comprehensive symptom assessments and facilitating care discussions through remote communication. Key responsibilities Conduct comprehensive symptom assessments and support members with serious illness longitudinally Facilitate end-of-life care discussions and coordinate care with healthcare teams Participate in case conferences and collaborate with non-clinical team members to address social determinants of health Required qualifications Active, unrestricted Registered Nurse (RN) license, with New York licensure preferred Degree in Nursing (ADN or BSN) required; BSN preferred At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice for oncology patients Preferred certifications include Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications Fluency in Spanish, both written and verbal, is preferred
