Empowering families for optimal wellness, the full-time remote New York Licensed Nurse Case Manager will assess member needs, develop individualized care plans, and coordinate medical services for high-risk populations in a telephonic managed care setting. Key responsibilities Assess the physical, psychological, and social needs of members and develop tailored care plans Monitor care plans and collaborate with healthcare providers to ensure effective service delivery Educate members on their conditions and treatment plans while facilitating access to community resources Required qualifications Current, unrestricted Registered Nurse (RN) license in New York State Three or more years of clinical experience in a Level III or above NICU CCM certification required within the specified timeframe Experience in a Case Management role is preferred Proficiency in data entry into databases or EHR systems
