To enhance patient care in the HV region, the full-time New York Licensed Complex Care RN will provide telephonic triage, chronic disease coordination, and nursing support across multiple sites, ensuring timely and effective care delivery while working in a hybrid environment. Key responsibilities Serve as a remote care-coordination resource, partnering with clinical teams to close care gaps and ensure continuity of care Conduct telephone medication reconciliation and structured post-discharge outreach to patients, addressing their needs and facilitating follow-ups Maintain accurate documentation of all coordination activities in the electronic medical record to ensure compliance and continuity of care Required qualifications Current, unencumbered Registered Nurse (RN) license in the State of New York BSN preferred; ADN considered Experience in care coordination, chronic disease management, or telehealth/triage strongly preferred Proficiency with electronic health records and telehealth technology CPR/BLS certification current
