New York Licensed Case Management Nurse

To support comprehensive case management services, the full-time New York Licensed Case Management Nurse will conduct assessments, develop care plans, and coordinate care across the healthcare continuum while ensuring compliance with regulatory standards. Key responsibilities Conduct comprehensive assessments to identify medical, behavioral health, and psychosocial needs Develop and implement individualized care plans in collaboration with members and interdisciplinary teams Coordinate care to promote quality outcomes and address barriers to treatment adherence Required qualifications Current and unrestricted Registered Nurse (RN) license; New York State RN licensure preferred Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience considered Minimum two years of nursing experience in case management or related clinical settings Experience in behavioral health case management or psychiatric nursing preferred Knowledge of care management principles and healthcare delivery systems
