New York Licensed Care Navigator

Dedicated to enhancing patient health, the full-time New York Licensed Care Navigator will support patients enrolled in chronic care management and remote patient monitoring programs, conducting virtual consultations and collaborating with care teams in a remote setting. Key responsibilities Conduct virtual consultations focusing on patient education related to chronic diseases Monitor and respond to remotely transmitted patient data, documenting communications in patient records Assist patients in setting up their monitoring devices and maintain ongoing communication to empower them in managing their health Required qualifications Current, valid, and in good standing New York Nursing License (LPN/LVN) 3+ years of experience in primary care, home care, or chronic care management Experience in remote monitoring and/or virtual care management programs (preferred) Ability to work in a HIPAA-compliant and distraction-free remote environment Reliable internet connection with minimum speed requirements
