New York Care Specialist

To support care management processes, the remote New York Care Specialist will execute non-clinical tasks such as data entry, telephonic outreach to members, and collaboration with clinical staff while working a shift from 11:30 am to 8:00 pm. Key responsibilities Perform accurate intake and data entry for Care Management requests and referrals Conduct telephonic outreach to members for program enrollment and support care interventions Collaborate with the interdisciplinary team to manage caseloads and update care plans for members Required qualifications Bachelor's Degree Medical Assistant certification preferred 1 - 2+ years of client-facing or data entry experience in a healthcare environment Proficiency in MS Office and knowledge of medical terminology Ability to work a flexible schedule, including weekends, nights, and holidays as needed
