New Mexico Licensed Care Coordinator

To enhance patient care coordination, the full-time salaried New Mexico Licensed Care Coordinator IV will facilitate interdisciplinary team collaboration, conduct comprehensive health assessments, and implement care strategies for clients with complex conditions in a remote setting. Key responsibilities: Facilitates a team approach with the Interdisciplinary Care Plan team to ensure effective and cost-efficient delivery of quality care Conducts in-depth health risk and comprehensive needs assessments, addressing various member needs Provides care coordination for members with chronic or complex conditions, implementing and monitoring strategies to improve health outcomes Required qualifications: Master's degree with 4 years of experience, or Bachelor's degree with 8 years, or Associate's degree with 9 years, or 12 years of relevant experience in lieu of formal education Proficiency in Microsoft Word, Excel, and Outlook Experience in analyzing trends using decision support systems Business management skills, including cost/benefit analysis and negotiation Knowledge of referral coordination to community and public/private resources
