To support the care of Dual Special Needs members, the full-time New Jersey Licensed SNP Care Manager will assess clinical and psychosocial needs, develop individualized care plans, and coordinate services while working remotely. Key responsibilities Assess members' needs and develop member-centered care plans based on clinical guidelines Coordinate care with healthcare providers and advocate for members to ensure access to necessary services Monitor healthcare services and track member progress, adjusting care plans as needed Required qualifications High School Diploma or GED required Minimum of three (3) years of clinical experience in home care, discharge planning, or case management Active, unrestricted New Jersey license as an RN, LPN, SW, or CSW Strong knowledge of case management standards and managed care concepts Experience working within the healthcare delivery system
