Responsible for performing RN duties and managing care transitions, the full-time New Jersey Licensed RN will assess clinical needs, coordinate care plans, and advocate for members while working remotely. Key responsibilities: Assess patients' clinical needs against established guidelines to ensure appropriate care levels Coordinate and implement care plans, monitoring services and negotiating fees as necessary Document patient care activities comprehensively and interact with multidisciplinary teams to ensure continuity of care Required qualifications: Active Unrestricted NJ RN License or active Compact License required High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree Minimum of two (2) years clinical experience required Proficiency in Microsoft Office products and knowledge of case/care management principles preferred Understanding of healthcare contracts and benefit eligibility requirements preferred
