To support patient care, the full-time New Jersey Licensed Oncology Case Manager will assess clinical needs, coordinate care plans, and advocate for members while working remotely. Key responsibilities: Assess patients' clinical needs against established guidelines to ensure appropriate care levels Coordinate and implement care plans, collaborating with multidisciplinary teams and external providers Monitor and evaluate care activities for effectiveness and appropriateness, advocating for members throughout their healthcare journey Required qualifications: Active Unrestricted NJ RN License or active Compact License required Minimum of two (2) years clinical experience and two (2) years healthcare payer experience High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree Knowledge of case/care/disease management principles and healthcare contracts CCM certification preferred for Clinical Advocate roles
