To support a new Longevity Program, the remote New Jersey Licensed Geriatric Nurse Practitioner will deliver initial assessments and ongoing medical follow-ups to older adults via telehealth, focusing on comprehensive geriatric assessments, chronic disease management, and cognitive health. Key responsibilities Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk Provide medical follow-up visits for chronic disease management and ongoing care Perform cognitive screening and staging, and manage GLP-1 therapy for appropriate patients Required qualifications Active, unrestricted NP license in New Jersey Board certification as an Adult-Gerontology NP or Family NP with a geriatric focus Active DEA registration Experience with Medicare documentation and E/M coding Comfort with telehealth and EHR documentation
