To support community-based housing initiatives, the full-time New Jersey Housing Specialist will develop and implement housing strategies for MLTSS/SNP/Medicaid members, manage housing resources, and provide education to staff while working remotely. Key responsibilities Develop a housing strategy for MLTSS/SNP/Medicaid membership Identify and manage resources for housing options and assist with member placements Provide education to care management and advocacy staff on housing resources and options Required qualifications High School Diploma/GED required Three years of experience assisting vulnerable populations in securing housing Two years of experience in a public agency or related social service field Familiarity with public and private housing resources and stakeholders Knowledge of Medicare/Medicaid and Managed Long-Term Services and Supports (MLTSS)
