New Hampshire Licensed Case Manager

Providing clinical social work interventions, the full-time New Hampshire Licensed Case Manager will assess treatment needs, develop and monitor treatment plans, and empower clients to achieve their goals, working onsite three days a week and remotely two days a week. Key responsibilities: Coordinate and provide safe, effective, and client-focused care across the continuum Monitor and evaluate client progression through case management plans, making necessary adjustments Educate and support patients in transitioning to community-based services Required qualifications: Master's degree in social work, Mental Health Counseling, or a related field Experience in a social service setting with knowledge of case management systems Ability to assess crisis situations and respond appropriately Strong documentation skills in accordance with Federal and State guidelines Effective communication skills with diverse individuals and backgrounds
