Credentialing timeframe: 60-90 days after receiving completed application with clean file

Ongoing dates will begin Feb when schedules are released

Shift Details
Monday-Thursday: 8:00 AM - 4:30 PM
Friday: 8:00 AM - 12:00 PM with admin time 12:00 PM - 4:00 PM
Monday-Friday schedule
Lunch break 12:00 PM - 1:00 PM (paid time)

Requirements
AANP or ANCC Certified
Arizona License or compact license with LOQ in hand at time of submission (NP)
DEA
Active ACLS, BLS - Highly Preferred - must be willing to obtain during credentialing
Clean malpractice/background
Local candidate - Preferred

Botox for migraines (if credentialed)

Read EEGs
