Needs Assessment Coordinator

To support client-focused care, the remote Needs Assessment Coordinator will conduct screenings on potential client calls, complete necessary documentation and reports, and manage on-call admissions and referral needs. Key responsibilities Conduct screenings on potential client calls and complete all associated documentation Prepare and submit required reports related to Needs Assessment and Referral Development Manage on-call admissions and handle referral and transportation needs during off hours Required qualifications High School Diploma or equivalent required; Bachelor's Degree in a Human Service field preferred Experience in a health or human services agency, preferably in a drug and alcohol setting Working knowledge of emotional, mental health, and drug & alcohol issues Ability to maintain confidentiality and demonstrate ethical conduct Participation in training and development activities to enhance job knowledge and skills
