Multistate Licensed RN Case Manager

To support optimal transitions of care, the remote Multistate Licensed RN Case Manager will monitor real-time 'Admit, Discharge, Transfer' notifications, collaborate with ED and outpatient care teams, and manage patient outreach and discharge coordination. Key responsibilities: Independently collaborates with ED and outpatient care teams to create individualized transition plans for members Acts as a clinical liaison with hospital staff to perform transition of care and care coordination for ED discharges Conducts outreach following ED discharge to confirm access and completion of discharge plans Required qualifications: Current, unrestricted Multistate RN license 4+ years of diverse clinical experience in caring for acutely ill patients 1+ years of experience working in the Emergency Department Knowledge of utilization management and discharge planning Ability to recognize early signs of clinical deterioration and identify patients at risk for ED return visits
