Providing virtual preventive and primary care services, the part-time Remote Telehealth Nurse Practitioner will conduct annual wellness visits, manage chronic conditions, and perform follow-up consultations for Medicare patients while working flexible hours from home. Key responsibilities Conduct virtual Annual Wellness Visits and evaluations for new patients Manage chronic conditions and support remote patient monitoring Document patient encounters accurately within the EHR and maintain compliance with telehealth requirements Required qualifications Active and unrestricted Nurse Practitioner license(s) Experience providing preventive and/or primary care services Licensed in three or more states, including Mississippi, Louisiana, Illinois, Georgia, or South Carolina Ability to commit at least 16 hours per week Experience using EHR systems and comfortable with telehealth technology
