Multi-State Licensed Telehealth NP

To deliver comprehensive virtual care, the part-time contract Telehealth Nurse Practitioner (NP) will conduct new patient evaluations, manage chronic conditions, and perform Annual Wellness Visits remotely, requiring a minimum commitment of 16 hours per week. Key Responsibilities Conduct Annual Wellness Visits (AWV) and E/M virtual visits Perform new patient evaluations and follow-up consultations Manage chronic conditions and support remote patient monitoring Required Qualifications Active, unrestricted NP license in three or more states (MS, LA, IL, GA, SC) Proven clinical experience in preventative and/or primary care, with telehealth experience preferred No history of involvement in a Malpractice Settlement Understanding of CMS credentialing processes Strong digital fluency and excellent EHR documentation skills
