To deliver exceptional diabetes care, the full-time salaried Multi-State Diabetes Care Provider will conduct virtual consultations, develop individualized treatment plans, and collaborate with an interprofessional team to enhance patient outcomes in a remote setting. Key responsibilities Conduct thorough assessments of diabetes patients through virtual consultations, including diagnostics and treatment planning Formulate accurate diagnoses and individualized treatment plans, including medication management and lifestyle modifications Collaborate with care team members to coordinate holistic care and ensure continuity of care for patients Required qualifications Master's or Doctoral degree with an active RN and APRN or PA license Minimum of 2 years of experience in diabetes management and education Proficiency in telehealth technology and electronic health records (EHR) Multi-state licensure with a willingness to obtain additional licensure as needed Experience working in outpatient clinical practice, ideally in an endocrinology or diabetes-focused clinic
