MSW Care Manager — Home Health & Hospice

Enhabit Home Health & Hospice is seeking a licensed MSW to administer social services for patients and to educate patients, families, and health care team members. The role conducts services in alignment with physician orders and the established plan of care (POC).

Applicants must hold a Master’s in Social Work, be licensed in the state, and have at least one year of health care social work experience. Proficiency with mobile devices and basic technology is required; Medicare home health
