Primary Vascular Care is a mobile specialty group serving long-term care facilities. The NP/PA conducts on-site assessments to determine appropriate patient treatment plans and reports to the Lead NP.
This role travels throughout the New York City area, performing history and physicals, ordering tests, coordinating care, educating families, and maintaining physician relations. Regular reports are completed timely to support clinical programs.
