Mobile Nurse Practitioner - Adult/Geriatric

Practice beyond the walls ofa traditional clinic as a full-time Mobile Nurse Practitioner serving patientsthroughout San Gabriel Valley and East Los Angeles, California. In this role,you will bring high-quality, relationship-based care directly to patientsthrough home visits, community-based care, and virtual appointments whileworking within a patient-centered, value-based care organization focused onimproving outcomes and supporting long-term health.

Our care model is focused onquality outcomes, preventive care, proactive interventions, and coordinatedpatient support rather than high visit volume. You will enjoy a high level ofclinical autonomy while working alongside an interdisciplinary team dedicatedto helping patients achieve better health outcomes.

Position Highlights

• Full-Time Schedule:Monday–Friday | 8:00 AM–5:00 PM
• Competitive Base Salary: $130,159-$195,238
• $10,000 Sign-On Bonus

Qualifications

• Active and unrestrictedCalifornia Nurse Practitioner license
• Active DEA license
• Current BLS certification
• Strong clinical assessmentand diagnostic skills
• Ability to practiceindependently while collaborating effectively with an interdisciplinary careteam
• Commitment topatient-centered, preventive, and value-based care
• Excellent communication andrelationship-building skills
• Comfortable providing carein home, community, and virtual settings
