Minnesota Licensed Triage Nurse

Providing telephone triage support for Assisted Living and Hospice care, the part-time Minnesota Licensed Triage Nurse will work remotely every third weekend from 7am to 3pm, ensuring high-quality patient care through assessment, documentation, and follow-up on patient needs. Key responsibilities Independently provide professional nursing services over the telephone, documenting symptoms, assessments, and advice given Follow up on high-risk situations and analyze data to develop solutions for improved service outcomes Maintain clinical standards and ethics while documenting efficiently in electronic health records Required qualifications Active Registered Nurse license in Minnesota, or ability to obtain within 60 days of hire Minimum of 1 year of care management experience in home care or hospice Strong clinical and relational skills with the ability to analyze situations and implement solutions Capability to manage multiple priorities and high-stress situations effectively Proficiency in Microsoft Office Suite
